Responsibilities

The Director of Staff Development/LVN promotes and restores resident's health by providing physical and psychological support and care to patients and their families. Responsible for providing nursing care to all patients in accordance with hospital policies and procedures. Understands the role of the vocational nurse as a part of the nursing team.

 

ESSENTIAL DUTIES AND RESPONSIBILITIES: 

1. Provides basic patient care by:
• Demonstrating competence in taking vital signs, body systems assessments, giving enemas, measuring I&O accurately and recording results accurately and legibly.
• Responding to patient needs and requests promptly.
• Demonstrating proficiency in administering medications using standard methods.

2. Ensures operation of equipment by:
• Demonstrating competency in the use of equipment and other medical supplies.
• Maintaining equipment inventories.
• Evaluating new equipment and techniques.

3. Establishes a compassionate environment by:
• Maintaining a positive and caring relationship with patients, families and co-workers.
• Caring for the geriatric patient demonstrating clinical skills and knowledge of the aging process.

4. Maintain a safe and clean working environment by:
• Complying with established procedures.
• Supporting and promoting safety standards, identifying risk situations and intervening appropriately.
• Ensuring that utility rooms are maintained in a neat and orderly fashion.

5. Protects patients and employees by:
• Adhering to infection control policies and protocols, medication and storage procedures and controlled substance regulations.
• Maintaining current knowledge of universal precautions in dealing with potential exposure to blood borne pathogens.

6. Advocates for patients by:
• Initiating and maintaining confidentiality, privacy, and safety for all patients.
• Monitoring CNA’s to ensure compliance with Hospital and HIPPA policies and procedures as they apply to privacy.

7. Ensures continuity of patient care by:
• Providing timely and accurate information to staff for the next shift.
• Monitoring staff performance to ensure compliance with all policies and procedures regarding patient care.

8. Participates in SNF staff development programs by:
• Conducting in-service trainings
• May assist in administering the annual competency testing program.
• Coordinating MDS activities and reviews.

9. Participates in SNF Quality Assurance activities by:
• Providing quality review reports.
• Facilitates interdisciplinary round/meetings to identify and resolve patient/family related issues that may affect the discharge outcome.
• Completing the TAR accurately to ensure timely reimbursements from Medi-Cal.
• Maintaining the Compliance Matrix (DHS Survey Preparation) ensuring that all information is available.
• Scheduling Care Plan conferences.

10. Assists with patient admissions and transitions to home by:
• Monitoring all admissions to ensure compliance with established procedures and protocols and conducting follow-up reviews for QA purposes.
• Identifying patient/family discharge needs and providing support services and/or referrals to support services and agencies as required.
• Ordering equipment for home use and initiating follow-up calls to monitor patient condition.

11. Acute Responsibilities:
• Primary responsibility of the acute inpatient. This includes all aspects of patient care, assessment, nutrition, hygiene, medication administration within the scope of the LVN practice and reporting to the RN.
• In the event there are not acute inpatients, assistance may be required by the RN in the Emergency Department &/or the LVN in the Skilled Nursing Facility. Assistance will vary depending on needs at any given time.
• Ordering and stocking of supplies. This will cover supplies used by the Emergency Department as well as those in Acute. Be sure to keep par levels up in ER Back up, check and restock supplies in ER daily keep aware of outdates. Check supply areas in Acute when in use.
• QA charts. Daily, check the charts from the previous day for completeness and correctness. Follow up with nurses needing to make corrections. Report repeated noncompliance to the DON.
• Employee Health Nurse. This will include new hire physicals, maintaining the employee health records, annual physicals, PPDs and immunizations and all records for the above mentioned.

12. Performs additional duties as may be assigned by SNF DON/ADON or Nursing Services DON.

Qualifications

Hours of Work: 3-12 hour shifts/week. Day and Night shifts available.

Education:

  • High School Diploma and/or GED equivalent.
  • Graduate of an accredited school of nursing for LVN. 


Experience:

  • 1 year preferred. 


License or Certification:

  • Current CA LVN License
  • Requirement of a DSD certification
  • CPR
